Application Process
There are separate steps to apply for Housing Services and Attendant Care Services at QUAD Inc. To ensure that your application is processed and added to the list, please fully review and complete the application(s) below. QUAD Inc. typically has a 12-18 month waiting list, so an apartment may not be immediately available.
Housing Services
As a HUD provider, there are specific procedures, policies and regulations that QUAD Inc. must follow. You must print the completed application, sign it in ink and mail it to QUAD Inc. at 5125 SW Macadam Avenue, #205, Portland, Oregon 97239.
